In order to make your treatment with us more affordable for you, and to assist you with the confusing maze of insurance paperwork, we will be happy to file your insurance for you. We are participating providers for Medicare, Blue Cross Blue Shield, United Healthcare and many other plans. The front desk receptionist will need to make a copy of your insurance card for your account records regardless of the type of insurance you have. Please make sure you have your insurance card with you at the time of your initial visit.
Even though we are glad to offer this service to our patients, we must remind you that your insurance is a contract between you and your insurance company. We will do our very best to verify your insurance benefits and determine your coverage, but ultimately your bill is your responsibility and you may be billed for a portion of your bill not paid by your insurance company. Some plans require a prior authorization from your primary care physician.
YOU WILL BE RESPONSIBLE FOR ANY “OUT-OF-POCKET” EXPENSES SUCH AS DEDUCTIBLES, COPAYS, AND COINSURANCE AT THE END OF EACH APPOINTMENT UNLESS A PRE-PAYMENT PLAN HAS BEEN MADE.
You will receive a statement each month if your account has a balance due. |
